This patient presented with a Class III malocclusion — a deficient upper jaw causing the lower front teeth to bite in front of the upper (anterior crossbite), combined with severe dental crowding in both arches. The patient was not willing to undergo jaw surgery.
In a Class III bite, the lower jaw sits forward relative to the upper. This can cause the front teeth to cross over — the lower incisors sit in front of the upper incisors instead of behind them. When the underlying cause is a structurally deficient upper jaw (maxillary hypoplasia), the gold-standard correction is surgical. When a patient declines surgery, orthodontic camouflage — carefully compensating within the teeth alone — can still achieve a functional and aesthetic result.

What was done
In camouflage treatment for Class III, extracting lower premolars allows the lower incisors to tip back and the lower arch to retract, effectively disguising the skeletal discrepancy. Upper extractions are generally avoided as the upper arch already needs to be as full as possible to compensate for the deficient maxilla. The result is a functional bite and an improved aesthetic profile — without surgery.
Anterior crossbite fully corrected. Upper and lower arches well-aligned. Crowding resolved. Stable Class I canine relationship achieved — all without surgical intervention.
